About the Role
As a Needs Assessor Mental Health, you will play a vital role in supporting people with mental health and addiction support needs to live as independently and meaningfully as possible. Working closely with tangata whaiora, whānau, community providers, and multidisciplinary teams, you will undertake comprehensive needs assessments, develop personalised support plans, and coordinate access to services that help individuals achieve their goals and improve their wellbeing.

About the Team/Service/ Location
Our team of dedicated professionals within the Mental Health and Addictions NASC service provides needs assessment and service coordination across Hawke's Bay. We are a collaborative and supportive team committed to person centred care, equitable health outcomes, and helping people access the services they need to thrive in their community.
Working in beautiful Hawke's Bay, you will be part of a service that values teamwork, continuous improvement, cultural responsiveness, and strong partnerships with whānau, service providers, and community organisations.

About you
As a Needs Assessor Mental Health, you will bring:
- Registration with a relevant New Zealand professional body and a current Annual Practising Certificate.
- Experience working within mental health, addictions, nursing, social work, occupational therapy, or a related health profession.
- Strong communication and relationship building skills with the ability to work collaboratively across services.
- A commitment to culturally safe practice and achieving equitable outcomes for Māori and diverse communities.
- Excellent assessment, care coordination, problem solving, and time management skills.
