- Salary band – $81,683 - $106,739 p.a pro rata + Kiwisaver

About the Role
This role will see you supporting patients in their rehabilitation journey. You will be involved in:
- Conducting thorough assessments of patients' health conditions and rehabilitation needs.
- Providing high-quality, person-centred nursing care, including administering medications, wound care, and monitoring vital signs.
- Assist patients with daily activities and mobility to promote independence and recovery
- Develop and implement individualised care plans in collaboration with the healthcare team, ensuring that each patient's unique goals and needs are addressed
About you
This role requires a nurse who is adaptive to change, resourceful, flexible to service needs and future development and a genuine passion for working in the community.
Within this position you will require:
- Comprehensive clinical assessment and problem-solving skills.
- Ability to work autonomously and collaboratively within the inter-professional team.
- Ability to direct and delegate to Allied Health Assistants
- Excellent communication skills to liaise with health professionals, both within the in-patient services and in the community setting.
- Recent experience of working with the older adults in acute, rehabilitation or community settings
This role covers a Monday to Sunday rostered shifts
You will need to be a NZ Registered Nurse with current APC.
