About the Role

As a Registered Health Professional (Social Worker or Occupational Therapist), you will play a key role in supporting older people and their whānau through their mental health and recovery journey.

Your work will include:

• Undertaking comprehensive assessments and developing tailored care or rehabilitation plans.
• Delivering interventions that promote independence, wellbeing, and quality of life.
• Supporting people living with dementia in both community and residential care (rest home) settings.
• Working collaboratively with whānau, carers, and other providers to improve outcomes.
• Contributing to meaningful, person-centred goals through therapeutic engagement and rehabilitation.
• Providing care coordination and supporting functional rehabilitation and environmental adaptations.
• Working as part of a collaborative, multidisciplinary team to deliver safe, effective, and integrated care.

Our Older Persons Mental Health service is dedicated to delivering holistic, culturally safe, and person-centred care across Hawkes Bay.

About you

You will bring:

• A relevant qualification in Social Work or Occupational Therapy with current NZ registration and APC
• Experience in mental health, ideally in community or older persons services
• Strong assessment, clinical reasoning, and intervention planning skills
• Confidence working within a multidisciplinary team and building effective relationships
• A full NZ drivers licence
• A commitment to culturally safe and equitable practice
